Safety of Plastic Surgery Patient on IV Anesthesia

Abstract
Purpose: Intravenous sedation becomes more and more popular in plastic surgery. It could replace general anesthesia in certain cases and provide more comfort & less economic demand to patients. But, there are some uncertainty about its safety.

Materials and Methods :From 2008 to 2009, one hundred and twenty six patients underwent intravenous sedation during their surgical procedures. Ketamine and midazolam was used in dose of 0.05 mg/kg and 0.5 mg/kg respectively via intravenous route. The patients were closely monitored with blood pressure monitor, pulse oximetry and electrocardiogram.

Results: One patient with diabetes and tendency of hypoglycemic episode experienced cardiopulmonary resuscitation and unanticipated admission to critical care unit. The medical staffs could not monitor her during full course of operation because of busy operation schedule and she was thought to have respiratory depression initially and as a result, coma. But after prompt cardiopulmonary resuscitation and meticulous care at ICU, the patient could recover her previous health status. Otherwise, there was no unplanned hospital admission or postoperative nausea and vomiting.

Conclusion:In summary, Intravenous sedation offers safe and effective means to make patients get prepared to the operation. But, they should be cautiously and continuously monitored throughout the total period of operation time and recovery phases.
